Text Message Notification Authorization

I authorize GEM Edwards Pharmacy to send text message notifications on my provided cell phone number.  Text message contents may include:

• When my prescription(s) is/are in process
• When my prescription(s) is/are being shipped
• Requests to contact the pharmacy
• Updates about your prescriptions

I understand that text communication sent via SMS is not always secure.  In order to receive secure communication that may contain personal health information, I understand that I must download the GEM Edwards App powered by RxLocal and enroll.  View instructions on downloading app and enrolling. 

I also understand that standard messaging/data rates may apply to messages, and that I am under no obligation to authorize GEM Edwards Pharmacy to send me text messages. 

I may opt out of receiving text message at any time by calling 1-866-552-5522.
