To ensure timely processing of appointments, we are requesting that you complete the COVID-19 Immunization Questionnaire prior to your scheduled appointment time. Click the link below to download and print.
Please remember to wear a proper face mask. Bring the completed questionnaire, driver’s license, and insurance card to your scheduled appointment time. If you are scheduling to receive your booster dose, YOU MUST bring your vaccination card with you!

Upon arrival, please park in the front visitor lot and proceed to the entrance (highlighted in red). You will be greeted in the lobby at your scheduled appointment time for a temperature check and then directed to the vaccine area.
